Perform hand hygiene and gather sterile catheterization supplies
Verify the order, patient identity, allergies, and indication for catheterization
Explain the procedure to the patient and obtain consent per policy
Provide privacy and position the patient supine with legs extended and thighs slightly apart
Drape the patient to expose only the genital area
Open the sterile catheter kit and maintain sterile technique
Put on sterile gloves
Prepare the catheter, lubricant, antiseptic swabs, drainage bag, and balloon syringe
Clean the glans penis and urethral meatus using sterile antiseptic swabs
If uncircumcised, retract the foreskin and keep it retracted during the procedure
Hold the penis upright at about a 60–90 degree angle
Apply sterile lubricant to the catheter tip
Gently insert the catheter into the urethral meatus
Advance the catheter until urine flows
Advance the catheter a little farther after urine return
Inflate the balloon with the prescribed amount of sterile water only after urine return and full insertion
Gently pull back until resistance is felt to seat the balloon at the bladder neck
Secure the catheter to the thigh or lower abdomen per policy
Attach the drainage bag and keep it below bladder level
Replace the foreskin if it was retracted
Dispose of supplies properly and remove gloves
Perform hand hygiene
Document catheter size, amount of balloon inflation, urine characteristics, patient tolerance, and any complications
