If you are a funeral director, ask the family for the deceased’s Social Security number and report the death
Contact the local Social Security office in person if needed
Provide the deceased’s full name

Provide the deceased’s date of birth
Provide the deceased’s date of death
Provide your name and contact information
Return any Social Security payments received after the date of death
Notify the bank if benefits were direct deposited
Keep a copy of the death certificate if requested
Ask the funeral home whether they will report the death to Social Security
