Confirm you have the legal right to revoke it
Prepare a written revocation of power of attorney
Include your name, the agent’s name, the date of the original document, and a clear statement revoking authority
Sign and date the revocation document
Notarize the revocation if required by your state or country
Notify the agent in writing that the power of attorney has been revoked
Retrieve and destroy copies of the original power of attorney if possible
Inform banks, healthcare providers, government agencies, and other third parties that relied on the power of attorney
Record the revocation with the same office where the original power of attorney was filed, if applicable
