Complete the U.S. passport application form DS-11
Gather proof of U.S. citizenship
Gather a valid government-issued photo ID
Bring a photocopy of the photo ID
Obtain a passport photo that meets U.S. requirements
Find a passport acceptance facility in Indiana
Apply in person if this is your first passport, you are under 16, or your previous passport was issued when you were under 16
Submit the application, documents, photo, and fees at the acceptance facility
Pay the required application fee and execution fee
Track the application status after submission
Receive the passport by mail when processing is complete
