Download Form 1095-A from your online account
Check your email for notices from the Marketplace about the form
Contact the Marketplace call center if you cannot access it online
Request a mailed copy if you did not receive one
Verify your mailing address in your Marketplace account
Contact your insurance agent or broker if they helped with enrollment
Keep the form with your tax records once you receive it
