Visit the nearest Ayushman Bharat empanelled hospital, CSC, or Ayushman Mitra desk
Carry your Aadhaar card and any other required identity or family documents

Provide your mobile number for OTP verification if required
Complete the e-KYC process
Submit the required details and documents
Wait for approval and beneficiary confirmation
Download or collect your Ayushman Bharat card after approval
Keep the card safe for future hospital treatment use
