Open Microsoft Word
Go to File
Select Options
Click Customize Ribbon
Check the Developer box
Click OK
Place the cursor where you want a form field
Use the Developer tab
Click Design Mode if needed
Insert a text content control for text entry
Insert a check box content control for checkboxes
Insert a drop-down list content control for selectable options
Insert a combo box content control for typed or selected options
Add placeholder text if needed
Set properties for each form field
Repeat for all required fields
Protect the form if you want users to fill only the fields
Go to Developer
Click Restrict Editing
Enable filling in forms
Start enforcing protection
Save the document
Use Save As if you want a reusable template
Test the form by filling it out
Share the document with users
