Use your legal name and current mailing address

Enter your Social Security number

Check the box for Single if you are unmarried and want withholding based on single status

If you have multiple jobs or a working spouse, use the Multiple Jobs Worksheet or the IRS Tax Withholding Estimator

If you have dependents, complete Step 3 and enter the qualifying amount

If you have other income not subject to withholding, enter it in Step 4(a) if needed

If you want extra tax withheld from each paycheck, enter the amount in Step 4(c)

Sign and date the form

Give the completed W-4 to your employer
