Complete Form DS-11 for a first-time passport, child passport, or if your prior passport was lost, stolen, or expired for too long
Gather proof of U.S. citizenship, such as a certified birth certificate, naturalization certificate, or previous U.S. passport
Gather a valid government-issued photo ID, such as an Illinois driver’s license or state ID
Bring a photocopy of the front and back of your ID
Get one passport photo that meets U.S. passport requirements
Find a passport acceptance facility in Illinois, such as a post office, county clerk, library, or courthouse
Make an appointment if the facility requires one
Bring all documents, the completed application, and payment to the acceptance facility
Pay the passport application fee and the execution fee, if required
Submit the application in person
Wait for your passport to be processed and mailed to you
Use Form DS-82 by mail if you are eligible to renew a passport
Use Form DS-11 in person if applying for a child under 16
Use Form DS-11 in person if you need a passport card, passport book, or both
Use expedited service if you need the passport faster
