Check your mailbox for a paper copy if you selected mail delivery
Contact the Marketplace Call Center at 1-800-318-2596 if you cannot access it online
Contact your state Marketplace if you enrolled through a state-based exchange
Make sure your mailing address and account information are up to date
Request a replacement copy if your form was lost or never received
