If you are the person named on the certificate, a parent, legal guardian, child, grandparent, grandchild, sibling, spouse, or domestic partner, you may request a certified copy
Complete the California birth certificate application form
Have the application notarized if requesting a certified copy
Provide a valid government-issued photo ID if required
Include the required fee
Submit your request to the California Department of Public Health, Vital Records, or to the county recorder/county vital records office where the birth occurred
Request by mail, in person, or through an authorized online service if available
If mailing, send the completed application, notarization, ID copy if needed, and payment
If requesting in person, bring the completed form, ID, and payment
If you are not an authorized person, request an informational copy instead
