Confirm the power of attorney document is valid and authorizes you to sign for the principal
Use the principal’s name first, followed by your name and your authority
Sign in the format: Principal Name by Your Name, as Attorney-in-Fact
Sign in the format: Principal Name by Your Name, POA
Sign in the format: Principal Name by Your Name, Agent
Include your initials only if the document or recipient requires them
Sign exactly as the POA document and the receiving party instruct
Add the date if required
Keep a copy of the signed document and the POA for your records
