Choose a blank form or a template
Enter a title for your form
Add questions using the plus button
Select the question type for each question
Mark questions as required if needed
Add sections, images, or videos if needed
Customize the form theme and colors if desired
Click the settings icon to adjust form options
Preview the form before sharing
Click Send to share by email, link, or embed code
Collect responses automatically in Google Forms
View responses in the Responses tab
Link responses to Google Sheets if needed
Edit the form anytime to update questions or settings
