Use your full name or company name

Include country (if mailing internationally)
Place the “From” address in the top-left of the page or letterhead
Use the same format as the address you would write on an envelope

If using email, include “From:” in the header (email clients handle this automatically)
If using a formal letter, align the “From” block to the left margin and leave a blank line after it before the date
