How to Report Disability Fraud?

Gather the person’s full name, address, date of birth, and any known claim or case number

Record the specific facts you believe are false or misleading

Note dates, times, locations, and names of witnesses

Keep copies of any documents, photos, messages, or other evidence

Report to the agency that handles the benefit or claim
