Turn off the PS4 console
Locate the small reset button on the back of the controller near the L2 shoulder button
Use a paperclip or similar tool to press and hold the reset button
Hold the button for about 5 seconds
Release the button
Connect the controller to the PS4 with a USB cable
Press the PS button to pair the controller again
