How To Read Eye Prescription?

Locate the prescription date and patient information on the page

Find the right eye (OD) and left eye (OS) sections

Read the SPH (Sphere) value for each eye

Read the CYL (Cylinder) value for each eye

Read the AXIS value for each eye (usually 0–180 degrees)

Read the ADD value (near-vision add) for each eye if present

Read PRISM and BASE (if present) for each eye

Read the PD (Pupillary Distance) or “Distance PD” and “Near PD” if listed

Note BC (Base Curve) if listed

Note DIA (Diameter) if listed

Note the lens type/material (e.g., single vision, bifocal, progressive) if listed

Check for additional notes such as “SV,” “PAL,” “Progressive,” “Toric,” or “Multifocal”

If the prescription includes multiple pairs (e.g., distance and near), match each set to the correct eye (OD/OS) and use-case

Record values exactly as written, including plus (+) or minus (−) signs
